Register With The Practice

IF YOU DO NOT COMPLETE BOTH PARTS OF NEW PATIENT REGISTRATION FORMS, WE CANNOT PROCEED WITH YOUR REGISTRATION REQUEST.

There are two stages to start the process to register with the practice:

  1. Fill in this quick online form
  2. You must also download and complete a New Patient Questionnaire (DOCX, 51KB) before you will be fully registered with the practice. 

If you do not complete and return the New Patient Questionnaire your registration will not be completed.

If you do not wish to use the online form in point one a paper version of the is form is available here.  Please be aware that you will still need to download and complete a New Patient Questionnaire (DOCX, 51KB).


Please fill in all paperwork with as much information as possible and bring it back to the surgery reception desk. Please ensure you give us your NHS number; this is on your medical card or you can phone your previous GP to ask for this number. To confirm residency, we will need to see a copy of a utility bill or bank statement which is no more than three months old and this will be copied and scanned into your patient record.

If you wish to register for online services we will need to see Photographic ID to do this.

When you return your forms to us an appointment to register will be given. Please avoid our busiest period, 8.30am to 2.00pm, when you return your paperwork. This will allow our reception staff to give you their full attention.

All patients 16 years old and over will be seen by our Health Care Assistant for a routine health check. This enables us to have a base line of your general health which we can check against should you become ill. The appointment lasts for 10 minutes.

Your paperwork will be reviewed by a GP who will confirm your suitability for registration. Then your information will be entered onto our computer system. This process can take up to four days before you are registered and able to see a doctor or nurse. It is best to telephone the appointment line to make your appointment and they will check if you have been registered.

You will be registered with Bellegrove Surgery but you may express a preference for a specific doctor, if they are on duty.
